What is the purpose of FedEx Claim Form?
A FedEx shipment may be lost or damaged. The purpose of the form is to notify FedEx of the loss or damages and claim compensation.
Who uses FedEx Claim Form?
The recipient, the sender or a third party can file the claim.
What documents should be attached to FedEx Claim Form?
The following documents should be provided:
- Duplicate of FedEx air waybill, FedEx Ship Manager® printout, FedEx Ground Pick-Up Record or delivery receipt.
- All documents relating to the evidence of value (copy of the invoice from supplier or vendor, copy of retail invoice or receipt, final confirmation screen in the case of online order with proof of payment, statement of non-repair or itemized repair invoice, appraisals, expense statement, or any other applicable documentation).
- If applicable, serial number(s) of merchandise.
- Inspection report, if applicable.
When is FedEx Claim Form due?
Claims for concealed loss and visible or concealed damage must be reported within 21 calendar days and all supporting documentation filed within 9 months of delivery date. Claims for non-delivery must be filed within 9 months of the committed delivery date.
How do I fill out FedEx Claim Form?
The claimant should provide the company name, address, phone, fax and e-mail of the sender and recipient, and fill the following blocks:
- Tracking or Freight Bill Numbers.
- Shipment Information. This block includes the following: ship date, number of packages, weight, FedEx control number, quantity of packages, item description, claimed amount, contents of shipment, description of damage to outer packaging, description of inner packaging, description of damage to contents, declared value, declared value for customs, merchandise value, FedEx pack & ship fee, freight charge, total claim/C.O.D. amount, customer remarks.
- Salvage. In this block should be filled with salvage contact, phone and fax.
- Claimant Information. The claimant signs and dates the form and gives his name, address, e-mail, phone and fax.
